Direct primary care (DPC) is a flat monthly membership with a doctor: unlimited visits, same-day or next-day appointments, and the doctor's cell number, with no insurance billing. On Long Island it runs from $135 a month per employee all in through Upsurance, which includes the practice's own rate plus our administration fee. The employer buys memberships; each employee picks the practice. Most companies pair it with a high-deductible plan for hospital coverage, or use it alone when a group plan is out of reach.

Pick a max per employee for a doctor and a dentist. The page shows which local practices fit.
They choose their own doctor and dentist from that list. No network cards, no claims.
Itemized per person. You pay what each person actually picked. Start or stop any month.
Office visits, sick visits, annual physicals, chronic-condition management, and direct messaging with the doctor. Labs and imaging are ordered by the doctor and paid separately at self-pay prices, which we also list.
Under the 2026 federal rules, a DPC arrangement at or below $150 a month per person is HSA-compatible. Several practices above fit under that. Check with your benefits advisor for your specific plan.
If their doctor is one of the practices on the list, yes. Otherwise they pick a new one; most are taking new patients now.
DPC covers primary care. For everything else, employees use their insurance or the self-pay prices on upsurance.co.
