What direct primary care is
Direct primary care (DPC) is a membership with a doctor. You pay the practice a flat monthly fee, no insurance involved, and get unlimited visits, same-day or next-day appointments, longer visits, and a direct line to the physician by phone or text. Most practices include basic in-office care and many include routine labs. Nothing is owed at the desk.
What it is not: hospital coverage, surgery, or specialists. DPC doctors refer you out for those, and most members pair the membership with either a high-deductible insurance plan for catastrophes or the cash-price labs, imaging, and urgent care listed on upsurance.co.

How to choose a practice
- Distance first. Unlimited visits only matter if the office is reachable. Every practice above lists its town.
- Check what's in the fee. Labs, EKG, in-office procedures, and telehealth vary. AMG bundles labs and minor procedures; Sona Via bundles hormone and gut-health work; Lewis Medical adds hospital coordination.
- Extras. A few practices charge a registration fee or a small sick-visit fee; they're noted above.
- Most offer a free consultation. Meet the doctor before you commit.
For employers: a doctor for every employee
Small businesses on Long Island and in NYC that can't justify a group health plan sponsor DPC memberships instead. The employer sets a monthly budget per employee; each employee picks the practice that fits; one invoice a month; the practice is paid its full rate. The employer's cost is the practice's price plus a $35 monthly fee that steps down with headcount, starting at $94 a month per employee.
